Ensure you have more than one copy of the death record; various institutions will require such documentation for settling an estate and youll find maintaining additional copies will help you speed up paperwork.

Below are four examples when providing a death certificate is required:

1. Transporting the body:

In most states, the funeral director will need a copy of the death certificate to legally transport the body to the funeral home and will file the death certificate with the courthouse for legal proof of death. The funeral director will also need a copy of the death record to apply with the health department for a burial transit permit.

2. Claiming a life insurance policy:

If your loved one had life insurance, the life insurance company will require an official certified death certificate as proof of death when a primary or secondary beneficiary submits a claim. Generally speaking, a secondary beneficiary is only allowed to make a claim if the person listed as the primary beneficiary has also died. In either situation, photocopies are considered unacceptable.

If the death was found not to be from natural causes (such as an accident or a homicide), you will also need a copy of the investigative report to be eligible for additional accidental death benefits. Check the fine print of the policy to see if this sort of coverage was provided.

3. Closing bank accounts:

Any existing checking and savings accounts will remain open and any applicable fees will continue to be charged as long as there is money in themunless you formally close the bank accounts. You will need to bring in a certified death certificate and fill out a Death Notification Form as supplied by the bank to officially close the account and withdraw or transfer any remaining funds.

If the account was a joint account with your name on it, the death notice is more of a formality to make the bank aware that no more deposits will come in under the deceaseds name beyond any related insurance settlements.

4. Transferring retirement benefits:

If you are named as a primary beneficiary on a 401(k), individual retirement account or company retirement plan, supply a certified death certificate to roll the benefits over to a retirement account in your name. If you decide to withdraw the money and place it into a checking or savings account, be aware that you may face a significant tax penalty.

The days and weeks following the death of a loved one can be very stressful. Settling an estate requires the submission of paperwork to each individual institution where an account is held by the deceased. Youll also need to provide paperwork when requesting a death-related service or benefit. Part of the paperwork requires providing documentation of the death. Automobile industry pioneer, John DeLorean, died Saturday in a New Jersey hospital by complications from a stroke.

DeLorean was born in 1925 in Detroit, Michigan to European immigrant parents. He received an education in automotive engineering and quickly rose through the ranks of Packard and later General Motors (GM). DeLorean was credited with the development of the Pontiac GTO, which helped introduce the era of “muscle cars”. By 1965, DeLorean led the entire Pontiac division, and four years later was promoted to the prestigious position of leading GM’s Chevrolet.

In 1973, DeLorean quit General Motors and started his own company, the De Lorean Motor Company. The company’s product was the DMC-12, an unusual car featuring an unpainted, stainless-steel exterior and gull-wing doors. The company started production in 1981 but failed less than two years later, having produced under 9,000 vehicles. Despite the company’s failure and the car’s dismal sales, the car itself gained a cult following after the release of the 1985 movie Back to the Future which featured the car as a time-travel machine.

DeLorean himself was in nearly as much trouble as his company. In 1982 he was arrested for attempting to sell $24 million worth of cocaine to undercover police, and after his company’s failure, he became involved in a multitude of lawsuits alleging investor fraud. Though DeLorean successfully resolved the cocaine case after claiming entrapment, his other legal cases would drag on until 1999, when he declared bankruptcy.

An advertisement for Dove beauty products has been viewed by well over three million people, without ever being on television. A copywriter from Ogilvy Toronto, the advertising agency that created a spot named “evolution”, uploaded the advertisement to video sharing website YouTube.

While the official upload of the ad itself has been viewed 1,119,262 times, there are dozens of copies of the ad on YouTube, adding to a minimum of 3,059,546 views. The official copy of the video is the website’s 12th most viewed this month, 53rd of all time.

Unofficial uploads have each received high levels of viewership, with 449595, 445322, 207906, 201670, 195265, 116501, and 102634 plays.

The agency did not originally intend to upload the video to YouTube, only display it on the company’s homepage. Staff member Tim Piper uploaded it to his account on October 6, about a week before it first got media coverage on Good Morning America.

The ad begins with a woman walking into a photo shoot. From there, she is primped and plucked by hair and makeup artists, then tweaked on a Photoshop-like program. The photo-manipulation is then posted on a billboard for the fictional “Easel Foundation Makeup” brand. Two young, teenage girls walk past, glancing at the board. “No wonder our perception of beauty is distorted” ends the ad in text, “Every girl deserves to feel beautiful just the way she is.”

The creative team for the ad included Tim Piper, Mike Kirkland, Janet Kestin, Nancy Vonk, directors T Piper (treatment and post production) and Yael Staav (live action) from Reginald Pike, Soho post production, Rogue editing, Vapor music, Gabor Jurina and Make-up: Diana Carreiro, and Reginald Pike.

The official French copy of the ad has only received 132 views, although it was only uploaded on November 2, 2006.

Wikinews has obtained a list of middle management teaching staff allegedly made redundant, or laid off due to restructuring, by Sunshine Coast Grammar School (SCGS), in Queensland, Australia. Sources say that those staff have been told that they can apply for new positions that have opened up.

The list, published on the SCGS alumni Facebook page, contains the names of twenty-two staff members, eight of which taught this article’s primary contributor. Seventeen positions are reportedly being opened up by the private Christian school, eight of which seem to significantly overlap the old ones.

The changes are, apparently, designed to get teachers back into the classroom. “We are not cutting subject choices and extracurricular activities, but retaining a student-driven curriculum that integrates with the new Australian Curriculum, in keeping with our commitment to teaching and learning opportunities,” said headmaster Nigel Fairbairn.

Fairbairn could not guarantee that any of the staff would be given a position in 2011. “That will depend on how many people on that list apply for new positions of responsibility and are successful,” he said.

“The Head of the School cannot guarantee the 21 staff a job at the School in the future, with many of the positions being advertised to external applicants,” said Terry Burke, secretary for the Independent Education Union of Australia Queensland branch (Queensland Independent Education Union, QIEU).

“There has been little or no consultation with affected staff, who should not have to reapply for their jobs,” he said. “Most of the proposed restructuring is highly questionable and places at risk the high quality education at the School.”

Some former students responded angrily to the news.

Four of the affected teachers “were the backbone of the school when [controversy surrounded founding headmaster John Burgess] happened,” a former prefect (student leader) said. “They got it through that crisis and this is the thanks they get.”

“People are angry and shocked,” they continued. “I am aware of at least 10 families who have said they will pull their children out of the school – it’s that bad.”

The student body has not ruled out protesting the schools plans. “It’s getting to that stage,” the former prefect said. “People are trying to look at it in an intelligent way but there is so much anger out there.”

Wikinews understands that Fairbairn attracted criticism when he was a head teacher in Christchurch, New Zealand, where a former student claimed that Fairbairn “replaced the open and welcoming culture … with the tyrannical and oppressive one.”

SpaceX Corporation launched its Falcon I rocket today from Kwajalein Atoll. Liftoff occurred at 6:00 PM PDT, following two previous launch aborts this week.

The first stage was completely successful, although there appeared to be some problems with the second stage. The sub-orbital flight reached an altitude of 300 km, and is on a traectory for re-entry. It was intended to go into orbit, but contact was lost before this could be accomplished

This is the second launch for the low-cost SpaceX rocket, and the first successful launch for the company. The previous launch had a successful lift-off, but failed 35 seconds into the flight and crashed near to the launch pad due to a fuel leak in the engine.

SpaceX called off the much-delayed inaugural launch of their new Falcon 1 rocket on Saturday from Kwajalein’s Omelek Island launch site. The intent was to launch the U.S. Air Force Academy’s FalconSat 2 satellite, which will monitor plasma interactions with the Earth’s upper atmosphere and magnetosphere.

The launch was delayed, then finally cancelled after an oxygen boil-off vent had accidentally been left open. The oxygen was unable to cool the helium pressurant, which then proceeded to evaporate faster than it could be replenished. A main computer issue, probably serious enough to cause a scrub on its own, was also discovered.

This long-anticipated flight was originally expected to be launched in January 2005, however a series of setbacks forced a series of delays, with the flight most recently scheduled to be in early 2006. It was intended to be launched from the Kwajalein atoll in the middle of the Pacific Ocean.

The maiden voyage was originally intended to launch from Vandenberg Air Force Base in California with a Naval Research Laboratory satellite and a Space Services Incorporated space burial payload.

On Tuesday the World Bank released the 2012 World Development Report on Gender Equality and Development. For Jamaica the report highlights a number of negative gender issues for the nation’s men.

The report claims that getting an education in Jamaica is viewed as primarily a female activity. This cultural attitude encourages males to leave school early. In 2008, girls outnumbered boys in secondary school by a ratio of 1.04:1. At the same time, boys were more likely to have to repeat a year of school. Only 16% of boys passed five or more Caribbean Secondary Education Certificate exams compared to 30% of girls. Boys outperformed girls only in vocational subjects and physics. The report cites four key challenges in boys’ development identified by a national programme. They are low self-esteem, limited future employment opportunities, lack of discipline, and masculine identities that eschew education.

A program in Jamaica uses cash incentives to encourage at-risk boys to stay in school; other countries like Pakistan use cash incentives to encourage girls to stay in school. Jamaica’s program has resulted an average increase in boys attending school by 0.5 days a month. At the same time, fathers are urged to become more involved with their childrens’ schooling and changes are being made to the curriculum to make it “more boy-friendly”.

Definitions of masculinity result in less employment opportunities and smaller earnings potential. The report claims Jamaican definitions of masculinity also encourage more risky behavior, and sexual behaviors valuing achievement and competence above intimacy. The report says these factors increase physical and sexual violence towards women.

Male mortality is increasing in Jamaica. The report cites crime and violence as causes.

The 1999 and 2000 year model Honda Civic SiR tops the list of Canada’s most stolen cars.

Consumer popularity also assures the cars will be popular with thieves. Its the second year in a row the Honda SiR has topped the list.

Rick Dubin Vice President of Investigations for the Insurance Bureau of Canada said “The Civics are easy targets.”

Dubin said that once stolen, the cars are most often sold to “chop shops” where thieves completely dismantle the vehicles. The automobile’s individual parts are worth more than the entire car.

The sheer numbers of the cars and their lack of theft deterrent systems make them thieves’ preferred choices.

1999 and 2000 Honda Civics do not come with an electronic immobilizer, however all Hondas from 2001 and onward are equipped with an immobilizer. Immobilizers will be mandatory on all new cars sold beginning September 2007. The devices enable an engine computer to recognize an electronic code in the key. If the code in the key and the engine don’t match exactly, the vehicle can’t be started.

In third place was the 2004 Subaru Impreza, while the 1999 Acura Integra came in fourth, with the 1994 Honda Civic rounding out the top five.

In sixth place, the 1998 Acura Integra, and the 1993 Dodge Shadow completed seventh.

When asked why early model vehicles are selected, he said that, “auto thieves continue to find it easier to steal older vehicles lacking an IBC-approved immobilizer. We’ve seen this trend developing for several years, and these results confirm it.”

Another Honda automobile, the 1996 year model Civic filled eighth place, with the 2000 German Audi TT Quattro in ninth.

The American 1996 Chevrolet/GMC Blazer rounded out the top ten.

None of the above cars had an electronic immobilizer.

London – A few hundred England fans weren’t watching the 2006 FIFA World Cup match against Sweden last night in a pub or at home, but instead had ventured out to their local cinema to see the game on the big screen. A number of Odeon cinemas nationwide have been using digital projection technology to screen the matches live with a high definition (HD) picture. HD broadcasts contain a greater level of detail than traditional TV broadcasts, meaning a sharper picture and better sound quality.

In the darkened auditorium of the cinema in Covent Garden, the audience (or should that be crowd?) were behaving almost as if they were at the stadium, singing along to the national anthem, cheering at the England goals and groaning at the Sweden chances. At times, chants being sung by the England crowd at the match were even picked up and sung along to by those watching the cinema screen like some kind of football karaoke contest.

Trailers before the match were replaced by a soundtrack of England anthems, both the successful and not-so-successful ones, and the traditional movie treat of popcorn was replaced by trays of beer (in plastic cups) being brought in by the punters. The cinema had cheekily listed the screening as being ‘directed by Sven-Göran Eriksson’ and as ‘starring Wayne Rooney (hopefully)’.

Despite a disappointing 2-2 draw, the audience seemed impressed with the experience. “I’m a bit short and so wanted to make sure I had good view without having to jostle around for position,” Amanda, from London, explained to me. “I also liked that it was non-smoking, and there was a fabulous atmosphere”. Sian, Caio and Laura, who lived locally, said they wanted to see the match on the big screen and commented on the excellent picture quality.

Other events that have been broadcast by the cinema chain include concerts by Robbie Williams and Elton John. Odeon Marketing Director Luke Vetere said “offering films is just one part of the cinema experience – our ambition is to offer guests the chance to watch other events they feel passionate about”. Watching football in the cinema is not a brand new event though, during previous World Cups such as in 1966, film footage from the matches was broadcast in cinemas after the event, providing a way for people to see the games in colour when TV broadcasts were in black and white.

Cinema screenings aren’t the only way that fans can watch the World Cup games in high-definition this year though, as both Sky TV and Telewest have been broadcasting the games in HD to viewers with a special set-top box. There have been trials with HD on the growing Freeview platform too, with a pilot group of a few hundred viewers in London. However, as any move to roll out HD on Freeview would use up extra space on the broadcast spectrum and would require viewers to buy a new set-top box, it seems unlikely that this will happen any time soon.
